All new students at CLCS this year as well as all K, 1st, 3rd, 5th and 7th grades will have their vision and hearing tested October 18th-22nd. This is a state requirement to help identify those with needs.
All of our students will be screened unless we have proof that they have been examined by a health care professional since May 1, 2010. If your child has had a vision or hearing screening since May 1, 2010, you must provide documentation of the examination to Nurse Linda by October 15th. If documentation has previously been sent for your child's records, please contact Nurse Linda and make her aware of it. Cost: Total screenings cost $10.00 ($5 for vision and $5 for hearing). This amount will be charged to your school account. STUDENTS WHO WEAR GLASSES MUST BRING THEM TO THE SCREENING. |
